We have received your application/medical history. A patient coordinator will contact you within 3 working days to answer general questions and guide you through the rest of the evaluation process. If you have not done so already, please send your most recent MRI disc (or X-Rays if you do not have an MRI) to our office (address below). If you wish, you may also submit images online. Your patient coordinator can assist you with online submissions.
